29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: Lack of understanding existed among some employees authorized to perform lockout/tagout work on the deboning machines, about which de-energization device to place a lock, that is, either the disconnect on the machine, which only de-energized the production chain and not the skinner rollers, or the disconnect on the ground floor, which de-energized the entire machine.
