Olmsted, Ohio 44070; the employer did not ensure employees working to install exterior sheeting on the West side of a gas station had proper access to the working height; employees were observed standing on the mid-rail of a scissor lift to gain access to the working height.

Olmsted, Ohio 44070; the employer did not ensure employees working on the roof of a gas station were protected from a fall of approximately 16.5 feet by utilizing a fall protection system.
