29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a)  4 Hanover Avenue, Florham Park, NJ.  Employee was exposed to penetrating eye trauma from flying particles and nails while using the coil nailer as no eye or face protection was worn.                                                    Violation observed  on or about 07/11/2018.

29 CFR 1926.1051(a):   Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  Instance a)  4 Hanover Road, Florham Park, NJ.  Employees working on the top plates of the upper floor framing were not using ladders but were climbing up and down the wood framing for access exposing themselves to approximately 8 foot falls to the floor below.                                       Violation observed on or about 07/05/2018.

29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Instance a)  4 Hanover Road, Florham Park, NJ.  Employees were exposed to falls of up to 28' to the ground below from the roof joists and from unguarded window openings where the window sill was 28 1/2" and not provided with an upper guardrail.  In addition, employees were exposed to falls of approximately 8 feet from the wood framing to the upper floor as fall protection methods were not being used.                                        Violation observed on or about 07/05/2018.  Instance b)  4 Hanover Road, Florham Park, NJ.  Employees were exposed to falls of 28' from the 6:12 sloped roof during roof sheathing activities.                                       Violation observed on or about 07/11/2018.
