29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 07/12/17, on various areas of a house roof at 669 French Road, Cheektowaga, NY: Three employees engaged in roofing work on a steep roof were exposed to a fall of up to approx. 20 ft. No fall protection system was utilized. Pitch of the roof varied in places from 6 in 12 to 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 07/12/17, on a roofing jobsite at a house, 669 French Road, Cheektowaga, NY: Employees used an extension ladder to access the roof from the ground. The side rails of that ladder extended 31 inches above the roof. NO ABATEMENT CERTIFICATION REQUIRED
