29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about June 20, 2018 at 23898 Cypress Grove, Lot 32, Gulf Shores, Al: The employer failed to ensure an employee used a ladder which extended 3 feet about the landing area to get on/off the roof during metal roof instillation which exposed him to fall hazards of 10 feet 10 inches to the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about June 20, 2018 at 23898 Cypress Shores, Lot 32, Gulf Shores, Al: The employer failed to ensure an employee installing metal roofing panels on a 6:12 and 10:12 pitch residential house used fall protection to prevent falls of 10 feet 10 inches to the ground.  Salvador Marmolejo Guerrero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1165807, citation number 1, item number 1 and was affirmed as a final order on September 9, 2016, with respect to a workplace located at McSara Ct, Spanish Fort, Alabama.
