29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. Jobsite: Employees are exposed to laceration and amputation hazards while using a miter saw to cut wood. An employee was seriously injured when cutting wood for a garden box. The employer does not provide training on safe operation of powered tools used in the course of maintaining residential facilities and building projects such as the garden box.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Jobsite: On or about June 22, 2018, the employer did not report to OSHA that an employee suffered a finger amputation and was hospitalized overnight.
