29 CFR 1926.1101(k)(9)(vi): Training for employees performing Class IV operations was not consistent with the Environmental Protection Agency (EPA) requirements for training of local education agency maintenance and custodial staff as set forth at 40 CFR 763.92(a)(1): a) Vehicle Maintenance Facility, 600 Church St, Norfolk, VA 23501: On or about July 18, 2018, and times prior thereto, the employer did not provide employees with training prior to Class IV operations, exposing employees to asbestos-related hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1101(k)(2)(ii)(A): Building and/or facility owners failed to notify prospective employers applying or bidding for work whose employees reasonably can be expected to work in or adjacent to areas containing asbestos-containing material (ACM) or presumed asbestos-containing material (PACM) of the presence, location and quantity of ACM and/or PACM at the work site: A) Vehicle Maintenance Facility, 600 Church St, Norfolk, VA 23501: On or about June 12, 2018, and times prior thereto, the building and facility owner did not notify a contractor of the presence, location and quantity of asbestos-containing flooring materials prior to contractor employees' removal of floor tiles and construction mastic containing up to 10.87 percent chrysotile asbestos, exposing employees to asbestos-related hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
