29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph. a) 1289 Walker Road, Dover, DE - On or about June 20, 2018 , the employer failed to ensure a guardrail system installed to be used for fall protection, complied with the requirements of the standard. Specifically where a workbench was used as a toprail on a pump jack scaffold, a midrail, screen/mesh, or intermediate member (such as a baluster or additional rail) was not installed, exposing employees to fall hazards approximately 13 feet above grade.
