29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a)  At the jobsite, on the upper roof, on or about 7/13/18:  Two workers were fitting membrane around a drain, outside the warning line, 10 feet from the lower level, and without a monitor present.  b)  At the jobsite, on the lower roof, on or about 7/13/18:  A worker moved guardrail to receive a 55 gallon drum of adhesive that was delivered to the roof by a fork truck.  This worker was not protected from falling 20 feet to ground.  c)  At the jobsite, on the upper roof, on or about 7/13/18:  Twelve workers were performing roofing work in close proximity to 15 unprotected skylights, and without fall protection of any kind.
