29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) At the jobsite, on the roof, on or about 7/13/18: Employees were installing air handling equipment on a low slope roof, 22 feet from ground, without fall protection of any kind.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the jobsite, accessing the roof, on or about 7/13/18: Employees used an extension ladder to access a roof where they were installing air handling equipment. This ladder did not extend above the upper landing.
