29 CFR 1926.451(e)(1): Scaffold platform more than 2 feet (0.6 m) above or below a point of access, was not provided with a portable ladder, hook-on ladder, attachable ladder, stair tower (scaffold stairways/towers), stairway-type ladder (such as ladder stands), ramp, walkway, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface.     a) Worksite, 10 Captains Neck Ln., Southampton, NY 11968, west side of residential building -  An employee was not provided with access to a ladder jack scaffold while working approximately 14 feet above the ground below; on or about 7/12/2018.     Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.     a) Worksite, 10 Captains Neck Ln., Southampton, NY 11968, Westside of residential building - An employee working from a ladder jack scaffold on the Northside corner of the residential building, approximately 14 feet above the ground below. The WERNER Portable Extension Ladder was set up at approximately 15 feet high and extended approximately 10 feet away from the residential building. The horizontal distance was approximately from the top support to the foot of the ladder was approximately one-half of the working length of the ladder, exposing employees to fall hazards; on or about 07/12/2018.     b)  Worksite, 10 Captains Neck Ln., Southampton, NY 11968, Westside of residential building - An employee working from a ladder jack scaffold on the Northside corner of the residential building, approximately 14 feet above the ground below.
