29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator had been involved in an accident or near-miss incident:    a) Warehouse- On or about July 5, 2018, the employer did not conduct refresher training, including an evaluation of the forklift operator's knowledge and skill, following an incident during which an employee attempting to reverse a powered industrial vehicle and hit a beam, sticking a water pipe.  No abatement certification or documentation is required for this item.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    a) Back Warehouse Area- On or about July 13, 2018, the employer did not ensure that all exit doors were kept free and unobstructed.  The exit route in the back of the warehouse that went through the storage room was blocked by material and boxes used to make displays.  No abatement certification or documentation is required for this item.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:  a) Warehouse- On July 13, 2018, the employer did not ensure that electrical equipment was free of damage and recognized hazards, such as accidental contact with electrical current. A plug connector on the electrical pallet jack was separating from the housing exposing the live copper wire.  No abatement certification or documentation is required for this item.
