The seat of the forklift had exposed springs which the operator would sit on during the forklifts operation. The forklift was a Komatsu forklift (Model: FG45T-6, S/N: 41911A).

No.: 8107W, S/N: P5-95, which was missing a tongue guard for the abrasive wheel on the right side (as one looks at the grinder from the front).
