29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Honda of America Manufacturing, Inc. - Anna, Ohio: On or about July 11, 2018, employees were exposed to injury from pinch points created by moving parts of the take-out clamp of the 251 die-cast machine in that it was not locked out prior to removing grease from the clamp.  The employer did not ensure that energy control application steps to control the hydraulic energy source were implemented prior to the servicing tasks; the employer did not:      - Turn off or shut down the equipment.  - Physically operate energy isolating devices to control energy sources and affix lockout device(s).  - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  - Verify that isolation and de-energization of the machine or equipment had been accomplished.    Honda of America Manufacturing, Inc. was previously cited for a violation this Occupational Safety and Health Standard, 29 CFR 1910.147(d), which was contained in OSHA inspection number 1291209, citation number 1, item 2a and was affirmed as a final order on March 5, 2018, with respect to a workplace located at 12500 Meranda Road, Anna, Ohio.

1910.147(e)(2)(i): Before energy was restored to the machine or equipment, the work area was not checked to ensure that all employees have been safely positioned or removed.     Honda of America Manufacturing, Inc. - Anna, Ohio: On or about July 11, 2018, the employer did not  check the work area to ensure that all employees were safely positioned or removed to a location protected from the hazard point. An employee working in direct proximity to the take-out clamp of the #1 die cast machine was exposed to a pitch point hazard in that the employee was not removed from the hazard or alternatively protected by alternative safeguarding measures during the task being performed.
