29 CFR 1926.501(b)(4)(ii): Each employee on walking/working surfaces was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about July 17, 2018, at a new building construction site located on the corner of N.W. 150th and Penn in Edmond, OK, employees were allowed to work in close proximity to uncovered holes at the site, exposing the employee to the hazard of falls.
