29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about July 17, 2018, at the trenching site located on the corner of Covell and Wellington Way in Edmond, Oklahoma, no protective devices were in place to protect an employee working in an excavation that was more than 5 feet in depth, exposing employees to the hazard of being crushed by falling soil. H and H Plumbing was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 982995, citation number 1, item number 1, and was affirmed as a final order on October 7, 2014, with a final abatement date of October 3, 2014, with respect to a workplace hazard located at 24th and Tecumseh in Norman, OK 73019.
