29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c): a) Open trench located in the vicinity of 36th Street West and Central Avenue: On or about July 17 2018 and at times prior, employees were exposed to cave-in hazards while installing a 12 inch water pipe in an unprotected excavation.
