29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      The employer did not ensure a guardrail system was installed where employees walked on the catwalk while cleaning oscillators for the centrifugal cutters and/or observing equipment operation on the mezzanine. There was an approximately nine inch wide section without guardrails and the oscillator height was 29 inches high, exposing employees to an approximately 12' fall hazard.     Del Monte Foods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.28(b)(1)(i) which was contained in OSHA inspection number 1247076, citation number 1, item number 1 and was affirmed as a final order on January 4, 2018, with respect to a workplace located at 1400 Plover Road, Plover, WI 54467.

29 CFR 1910.28(b)(6)(i): (i) Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard:     The top railing along the back-side of the centrifugal bean cutters on the mezzanine was 59 inches high and the remainder of the requirements in paragraph (b) were not implemented to prevent a fall hazard to employees walking on the walk-way onto a moving conveyor and/or the floor below.     Del Monte Foods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.28(b)(1)(i) which was contained in OSHA inspection number 1247076, citation number 1, item number 1 and was affirmed as a final order on January 4, 2018, with respect to a workplace located at 1400 Plover Road, Plover, WI 54467.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:     a) On the Bean Dock, employees were exposed to in-running nip points created by the rotating machine rollers and the out-feed metal chute frame on the small piece remover. The guarding in place did not prevent employee exposure to the in-running nip point created between the rotating roller and the chute frame.     b) In the production maintenance shop, the unused portion of the metal cutting blade on the Ellis horizontal band saw was not guarded to prevent inadvertent employee exposure to the rotating serrated blade.     c) On or about July 18, 2018 the rotating components in the ten 61H and 40P seamer machines on the canning lines were not guarded to prevent employee exposure to rotating parts, pinch point and/or crushing hazards. No positive means to prevent hinged covers from staying closed against the seamers was provided and/or there were openings near the outfeed conveyor exposing the rotating turret, not guarded to prevent employee exposure to the hazardous area.       d) On or about July 18, 2018 on the Bean Dock, employees were exposed to in-running nip point hazards created by the conveyor belt and rollers on the bean conveyor feeding the small piece remover.       e) On or about July 18, 2018 on the Bean Dock, employees were exposed to in-running nip point hazards created by the conveyor belt and the rollers on the bean conveyor feed by the bean sorting table.       f) On or about July 18, 2018 on the Bean Dock, employees were exposed to the in-running nip point hazards created by the conveyor belt and the rollers on the overhead end conveyor in the UBR department.       g) On or about July 18, 2018 on the Bean Dock, employees were exposed to the in-running nip point hazards created by the conveyor belt and the rollers on the bean conveyor perpendicular to bean sorting table.      h) On or about July 18, 2018 on the Bean Dock, employees were exposed to the in-running nip point hazard created by the rotating sorter roller at the northeast corner of the upper work area.       i) In the Warehouse, employees exposed to an unguarded in-running nip point hazard from the sag in the conveyor belt near station 6.       j) On or about July 18, 2018 in the Warehouse, employees were exposed to unguarded moving components on the packaging machine. The male portion of the interlock switch was inserted into the female portion and not connected to the hinged door.    Del Monte Foods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(1)  which was contained in OSHA inspection number 1247076, citation number 1, item number 3a and was affirmed as a final order on January 4, 2018, with respect to a workplace located at 1400 Plover Road, Plover, WI 54467.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself:     a) On or about July 18, 2018 the perforated sheet metal guards around the canning line #13 infeed portion, rotating seamer components and/or the outfeed portion were not secured and/or affixed to the machine during normal production.     b) On or about July 18, 2018 the perforated sheet metal guards around the canning line #12 infeed portion, rotating seamer components and/or the outfeed portion were not secured and/or affixed to the machine during normal production.     c) On or about July 18, 2018 the  perforated sheet metal guards around the canning line #11 infeed portion, rotating seamer components and/or the outfeed portion were not secured and/or affixed to the machine during normal production.     d) On or about July 18, the perforated sheet metal guards around the canning line #10 infeed portion, rotating seamer components and/or the outfeed portion were not secured and/or affixed to the machine during normal production.     e) On or about July 18, the perforated sheet metal guards around the canning line #1 infeed portion, rotating seamer components and/or the outfeed portion were not secured and/or affixed to the machine during normal production.
