29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: Archwood Protections in Conley GA: Stairs to the Chrom (rail) car area were not maintained in a safe condition, the landing area to access the top part of the chrom car was a wooden pallet with holes 2-3 inches in width and a gap of approximately 3-4 inches between the landing and the first step creating tripping hazards on or about July 7, 2018.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: Archwood Protections dba Lonza in Conley GA: On or about 6/28/18 an employee was exposed to chromic acid while sparging a Chrom rail car and was not wearing the proper PPE to protect his body or face from the hazardous mixture result in severe chemical burns to face and body on or about 7/17/18.
