29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. A) Lakeside Interior Contractors Inc. on or about June 26, 2018, at the address of 108 Whittlesey, Norwalk, OH: The employer failed to report an employee hospitalization (admittance) following a worksite incident (fall from a height). The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of employees hospitalization within 24 hours of admittance. The injured employee was admitted on June 26, 2018, for related in-patient treatment. The employer failed to contact OSHA between the time of admittance and the ER referral received by the Toledo AO on July 10, 2018.
