Location: 320 West 15th Street, New York, NY. On or about 7/17/2018, a) An electrical breaker panel was not installed with a cover. Live electrical conductors were exposed to accidental contact. b) Flexible cords entering the breaker panel were not installed with the proper fittings. Live electrical conductors were exposed outside of the panel box.
