29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a) On or about July 18, 2018, at the jobsite, an employee installing lath mesh from the second level of a frame walkthrough scaffold exposed to a 14 feet fall hazard in that the scaffold was not fully planked.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.    a) On or about 07/18/2018, at the jobsite, employees were exposed to a fall hazard while standing/working from the metal frame scaffold with a height to base width ratio of 4.67 to 1. The scaffolding was not restrained from tipping.
