29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a top rail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a top rail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart.  a) On or about July 16, 2018, at the east side of the commercial building, the second tier of supported scaffold used by employees to plaster stucco on the wall was missing guard rail components, exposing the employees to a 12 foot fall hazard.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:   a) On July 16, 2018, at the east side of the commercial building, employees plastering stucco were exposed to fall, collapse and struck by hazards, in that, the framed scaffolding system 3 tiers high by 4 columns across was missing cross bracing on the exterior side first level on the 1st, 2nd and 3rd columns, on the interior side on the first level 4th column and on the four columns on the 2nd and 3rd levels.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) On or about July 16, 2018, at the east side of the commercial building, the third level of the supported scaffold used by employees to plaster stucco on the wall was not fully planked, exposing the employees to an 18 foot fall hazard.   b) On or about July 16, 2018, at the east side of the commercial building, the second level of the supported scaffold used by employees to plaster stucco on the wall was not fully planked, exposing the employees to a 12 foot fall hazard.  IDV Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1257620, citation number 1, item number 1 and was affirmed as a final order on March 30, 2018, with respect to a workplace located at 4445 Casella Tampa, FL 33601.
