This was most recently evident during the inspection conducted on July 18, 2018 at a worksite located at 13210 Centennial Rd, Suite 4, Omaha, NE 68138. Two employees were using a forklift to load palletized shelving material onto the existing storage racks in the warehouse. One employee and the palletized shelving material were elevated to approximately ten or eleven feet. The employee fell to the ground breaking his left wrist.
