29 CFR 1910.28(b)(1)(I): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:      The employer is failing to ensure employees are protected from fall hazards associated with being elevated on the forks of a forklift.  This was most recently evident during the inspection conducted on July 18, 2018 at a worksite located at 13210 Centennial Rd, Suite 4, Omaha, NE 68138.  Two employees were using a forklift to load palletized shelving material onto the existing storage racks in the warehouse.   One employee and the palletized shelving material were elevated to approximately ten or eleven feet.  The employee fell to the ground breaking his left wrist.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    The employer is failing to protect employees from fire hazards in that the fire exit was obstructed.  This was most recently evident during the inspection conducted on July 18, 2018 at a worksite located at 13210 Centennial Rd, Suite 4, Omaha, NE 68138.  The employer has employees working in the warehouse portion of the facility where both east end emergency exit doors were blocked by pallets of mortar and stucco.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.     The employer is failing to properly train employees on how to operate the powered industrial trucks and recognize the struck by or caught under hazards associated with their operation.  This was most recently evident during the inspection conducted on July 18, 2018 at a worksite located at 13210 Centennial Rd, Suite 4, Omaha, NE 68138.  The employer has warehouse employees operating a forklift without providing the formal instruction portion of the training as required by the standard.
