29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: On or about July 18, 2018, employees performing masonry installation operations on a single-family home from a supported scaffold were exposed to fall hazards of approximately 15.6 feet when the employer failed to ensure the working level of the scaffold was fully planked. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about July 18, 2018, employees performing masonry installation operations on a single-family home from a supported scaffold were exposed to a fall hazards of approximately 15.6 feet when the employer failed to ensure a personal fall arrest system or guardrail system was used. No abatement documentation or certification is required for this item.
