29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On July 18, 2018, there were no frequent and regular inspections being performed by a competent person in that:    (a) employees were performing work from a ladder jack with unprotected sides and edges and not utilizing a fall protection system at fall heights greater than 11 feet and 12 feet    (b) employees were not provided an access ladder or equivalent safe access (the ladder jack platform, and platform support brackets, were positioned on the 'climb' side of the portable ladders used for platform support)    (c) employees were using the ladder jack support ladders as access to the upper platform, with the platform (and platform support brackets) positioned on the 'climb' side of the portable ladders     (d) employees were not provided with falling object protection in that the building front entrance (immediately below the ladder jack upper platform) was not barricaded to prohibit employees from entering the hazardous area

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On July 18, 2018, located on the north side of a residential structure, employees were performing work from a ladder jack with unprotected sides and edges and not utilizing a fall protection system. Potential fall heights were measured to be greater than 11 feet and 12 feet above the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    The employer did not provide a training program for employees that were performing work from a ladder jack while not utilizing a fall protection system and exposed to fall heights greater than 11 feet and 12 feet above the ground below.
