29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards. On or about July 18, 2018, the employer did not ensure employees engaged in roofing work activities who were working at a height greater than six (6) feet above ground level, were provided fall protection training. This exposed the employees to the fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about July 18, 2018, employees engaged in roofing work activities had accessed the roof and were working from portable ladders which did not extend at least three (3) feet above the roof's landing in order to prevent the ladder from slipping. This exposed employees to fall hazards.
