On or about July 18, 2018, an employee engaged in siding work activities and working from a height greater than six (6) feet above ground level, was not protected by using any method of fall protection. This exposed the employee to fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards. On or about July 18, 2018, the employer did not ensure an employee engaged in siding work activities and who was working at a height greater than six (6) feet above ground level, was provided fall protection training. This exposed the employee to the fall hazards.

29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person. On July 18, 2018, an employee engaged in siding work activities was working from a man-made scaffold. The scaffold was not constructed in accordance with the design by a qualified person; therefore, the scaffold design lacked the required capacity ratings, safety factors and stability assurance. This exposed the employee to fall hazards.
