The section of ladder used did not have slip-resistant feet and was missing a rung. The employee working on the scaffold was exposed to fall hazards of approximately 15 feet due to possible scaffold collapse while working on the ladder-jack scaffold which was on the lower roof, on or about July 19, 2018.
