29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) 1000 East Mountain Road, Wilkes Barre, PA: An excavator operator was attempting to remove soil from a trench and not determine the exact location of the partially uncovered gas main line, resulting in the excavator making contact with the gas line and causing a leak, exposing the operator and other employees in the vicinity to a fire and explosion hazard, on or about July 19, 2018. ABATEMENT CERTIFICATION REQUIRED
