29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to notify OSHA of work-related fatalities within eight hours. This condition most recently occurred on or about, July 16, 2018 at 5901 Swope Parkway, Kansas City, Missouri 64130 when an employee died from a work-related incident at approximately 4:00 a.m. OSHA learned of the incident at approximately 10:30 a.m. on July 17, 2018.
