29 CFR 1910.106(e)(2)(ii)(b)(2): The quantity of Category 2, 3 or 4 flammable liquid located outside of an inside storage room or storage cabinet in a building or in any one fire area of a building exceeded 120 gallons:  (a)  NORTH EXTERIOR OF PAINT KITCHEN, BUILDING 350 - The employer staged for use three (3) 55 gallon drums of Tan 686A paint (category 2 flammable liquid) and two (2) 55 gallon drums of Epoxy Hardener Part B (category 3 flammable liquid) near the exterior of the North Side of the paint kitchen.  Also staged in the near vicinity were two (2) 55 gallon drums of MIL-T--81772b T1 Polyurethane Reducer (category 2 flammable liquid) and three 55 gallon drums of liquid paint waste (category 2 flammable liquid).  The total quantity of combined category 2 and category 3 flammable liquids not properly stored totaled 550 gallons, on or about July 19, 2018.  No abatement certification or documentation required.

29 CFR 1910.106(e)(2)(iv)(a): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were not kept in covered containers when not actually in use:    (a)  BUILDING 350, PAINT KITCHEN - Employees performed paint kitchen operations in the paint kitchen structure, and the small steel thinner pot that was used to supply the polyurethane reducer (category 2 flammable liquid) for flushing the paint pump that was used to pump Tan paint into the pressurized paint pot was not kept covered when not in use, on or about July 19, 2018.  (b)  BUILDING 350, PAINT SPRAY BOOTHS - Employees performed painting operations in the paint spray booths.  Employees routinely filled pot liners with polyurethane reducer from a 55 gallon drum and placed the liner in the spray booth without a cover where it would remain under the spray cart until ready to be used for flushing the spray guns, on or about July 19, 2018.  No abatement certification or documentation required.

29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:  (a)  BUILDING 350, PAINT KITCHEN - The employer did not ensure that employees utilized non-sparking tools to remove and install bungs and also the syphon connection in the tops of the 55 gallon drums of paint and paint related materials used in the paint kitchen, on or about July 19, 2018.  No abatement certification or documentation required.

29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container are electrically interconnected:  (a)  BUILDING 350, PAINT KITCHEN - Employees used Sherwin Williams R91K00020 MIL-T-81772B T1 Polyurethane Reducer (Category 2 flammable liquid, flashpoint 37.4F, boiling point 172.4F) to flush and clean the pump after refilling the Tan paint tank, and the dispensing tube from the pump was not bonded to the small thinner pot that the reducer was circulated through and stored in, on or about July 19, 2018.  (b)  BUILDING 350, PAINT KITCHEN - Employees transferred Sherwin Williams R91K00020 MIL-T-81772B T1 Polyurethane Reducer (Category 2 flammable liquid, flashpoint 37.4F, boiling point 172.4F) to the small thinner pot using a diaphragm pump, and he dispensing tube from the pump was not bonded to the small thinner pot that the reducer was being transferred to, on or about July 19, 2018.  No abatement certification or documentation required.

29 CFR 1960.59(a): The Agency did not provide specialized job safety and health training appropriate to the work performed by the employee:  (a)  PAINT KITCHEN, BUILDING 350 - The employer did not ensure that employees received appropriate training to operate the paint kitchen located in building 350, on or about July 19, 2018.  (b)  PAINT KITCHEN, BUILDING 350 - The employer did not ensure that employees were trained to correctly ground and bond paint kitchen equipment, including but not limited to diaphragm pumps, 55 gallon drums of paint and paint related materials, and pressurized paint pots, on or about July 19, 2018.  (c)  PAINT KITCHEN, BUILDING 350 - The employer did not ensure that employees were trained to transport 55 gallon drums of flammable liquids and load the drums into the paint kitchen area located in building 350. A forklift equipped with a clamp device was sometimes used to place full drums of product onto the wheeled drum cradles and at other times, employees transported drums using wooden pallets, followed by manual force to move the drums into position, on or about July 19, 2018.  No abatement certification or documentation required.
