29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Instances A: Employees were exposed to electrical shock/burns due to a three prong plug that was not repaired in accordance the listing and labeling requirements. Instance B: Employees were exposed to electrical shock/burns due to STJW 16-3 extension cords that were used to provide electrical service to lighting and receptacles on workbenches. The cords were connected to an electrical box by a clip. The clip was not used in accordance with listing and labeling requirements.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installation(s) 600 volts, nominal, or less were used in prohibited manners: Note: Temporary electrical power and lighting installation(s) may only be used as specified by 1910.305(a)(2)(i)(A) through 1910.305(a)(2)(i)(C). Employees were exposed to electrical shock/burns due to the use of temporary electrical power and lighting on work benches. The benches were serviced by flexible 16-3 cords for approximately 1 year.
