29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a)  Buckeye Building & Home Improvement, LLC, on or about July 19, 2018, at the address of 2149 Airport Hwy, Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Buckeye Building & Home Improvement, LLC, on or about July 19, 2018, at the address of 2149 Airport Hwy, Toledo, OH: The employer did not ensure employees were protected from falls while removing old and installing new roofing material atop a residential type structure.  Employees operating in this condition were exposed up to a 10 ft. fall to the ground below.
