29 CFR 1910.266(d)(6)(i): Employees were not spaced and the duties of each employee were not organized so that the actions of one employee would not create a hazard for any other employee:    On or about July 19, 2018, employees were not adequately spaced so as to prevent struck by and crushing hazards. One employee utilizing a Palfinger A-Line Hydraulic Crane with a grappling attachment loaded cut logs from the ground into an open top container mounted on a 2017 Peterbuilt 567 double axle commercial truck.  Another employee had stored a gasoline powered chain saw into a fixed storage compartment mounted on the Peterbuilt truck below the open top container.  When the first employee loaded a ten foot log into the open top container, the log struck the upper side of the container, dislodged from the grapple, and fatally struck the second employee, who either had not left the work area, or had exited and reentered the work area.
