Powered industrial truck operators did not receive initial training in accordance with paragraph (l) of this section:  Employees were exposed to struck-by hazards.  The employee operating power industrial truck GEHL RS8-42 and employees working in the area were exposed to struck by hazards.   The employee operating fork lift at the commercial construction site had not received fork lift operator training.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  Employees were exposed to struck by hazards.  Workers engaged in construction work at the commercial jobsite were exposed to struck by hazards in that forklift GEHL RS8-42 did not have a back up alarm and was not taken out of service for repairs.
