The cylindrical bar front guard was not adjusted, and presented a 1.0 in vertical gap and a 2.75 in. horizontal distance to clamp rods.     f) Shop - On or about 07/16/18 and at times prior thereto employees operated the Haco hydraulic shear. The back of the machine was not guarded.     g) Shop - On or about 07/16/18 and at times prior thereto employees operated the Kalamazoo vertical band sander. The guards were removed.     h) Shop - On or about 07/16/18 and at times prior thereto employees operated the Johnson Notcher Machine #2. The brake drum was not guarded.     i) Shop - On or about 07/16/18 and at times prior thereto employees operated the wire machine. The rotating vertical spools and the moving wire were not guarded to prevent accidental contact with nip points and shear/crush points.
