Jack Gibson Construction Company, 8460 Tamarack Road, Meadville, PA  16335, Tamarack Lake, Dam B, on or about July 19, 2018:  Employees were utilizing flexible cords and electrical powered equipment including, but not limited to, a chest freezer.  A flexible cord was also powering the company's construction trailer.  The flexible cords and electrical powered equipment were not used in conjunction with GFCI and/or an assured equipment grounding conductor program.

The ladder was used in a matter in which it was not designed in that the employee would have to lean to one side to egress the ladder.
