At the jobsite located at 21517 W 98th St., Lenexa, Kansas:  Employees engaged in residential roofing activities using a pneumatic nailer were exposed to struck-by hazards in that eye protection was not provided.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsite located at 21517 W. 98th St. Lenexa, Kansas:  Employees engaged in residential roofing activities were exposed to fall hazards of approximately 10 feet and 6 inches from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing.
