On or about July 20, 2018, employees were exposed to a fall hazard while using the stairway to the shear lockout tagout area, as the guardrails had been damaged.

29 CFR 1910.28(b)(11)(i): (i) Each employee exposed to an unprotected side or edge of a stairway landing that is 4 feet (1.2 m) or more above a lower level is protected by a guardrail or stair rail system; 800 Shear Area stairway. On or about July 20, 2018, and at times prior thereto, employees were exposed to a fall hazard at the landing platform of up to approximately 10 feet to the ground below, as there were no guardrails on the south side.

On or about July 20, 2018, while the employer had trained employees in the specific lockout tagout procedures, the employer's written lockout tagout procedure on the Riverside Engineering shredder was not sufficiently detailed.

29 CFR 1910.215(a)(4): Abrasive wheel work rest was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: In the 800 Area. On or about July 20, 2018, employees were exposed to a struck-by hazard while grinding off bolts on the DeWalt bench grinder, Model DW 785, as the distance from the abrasive wheel to the work rest exceeded one-eighth inch. Monterrey Iron and Metal Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 1201000, citation number 1, item number 2-a and was affirmed as a final order on 3/20/2017, with respect to a workplace located at 2300 Frio City Road, San Antonio, TX 78226.

On or about July 20, 2018, employees were exposed to a struck-by hazard while grinding off bolts on the DeWalt bench grinder, Model DW 785, as the distance from the abrasive wheel to the tongue guard exceeded one/fourth inch. Monterrey Iron and Metal Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1201000, citation number 1, item number 2-b and was affirmed as a final order on 3/20/2017, with respect to a workplace located at 2300 Frio City Road, San Antonio, TX 78226.
