29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  On or about July 20, 2018, employees were potentially exposed to eye injuries while conducting tasks such as, but not limited to, set-up operations on machinery and the employer did not require the use of prescription safety glasses and or eye protection that could be worn over the prescription glasses as required by company's policy and hazard assessment.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
