29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) On or about August 1, 2018, the employer failed to record an illness on the OSHA 300 Log and failed to fill out a 301 Incident Report when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced on or about July 7, 2018. b) On or about August 1, 2018, the employer failed to record an illness on the OSHA 300 Log and failed to fill out a 301 Incident Report when an employee, delivering a package, suffered from and received treatment for a dog bite experienced on or about May 18, 2018. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.4(a), which was contained in OSHA inspection number 950338, citation number 2, item number 1 and was affirmed as a final order on February 11, 2014, with respect to a workplace located at 401 W. Lexington Ave. El Cajon, CA 92020. Or, "IN THE ALTERNATIVE" 29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: a) On or about August 1, 2018, the employer failed to record an illness on the OSHA 300 Log and failed to fill out a 301 Incident Report when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced first on or about July 7, 2018. b) On or about August 1, 2018, the employer failed to record an illness on the OSHA 300 Log and failed to fill out a 301 Incident Report when an employee, delivering a package, suffered from and received treatment for a dog bite experienced on or about May 18, 2018. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.29(b)(3), which was contained in OSHA inspection number 1075844, citation number 3, item number 1 and was affirmed as a final order on September 8, 2016, with respect to a workplace located at 616 W. 2nd St. Ottumwa, IA 52501.

On or about June 29, 2018, an employee, working in extreme heat conditions, experienced symptoms of heat related illness resulting in an in-patient hospitalization and the United States Postal Service did not notify Federal OSHA within the required timeframe. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.39(a)(2), which was contained in OSHA inspection number 1027982, citation number 1, item number 1 and was affirmed as a final order on March 12, 2015, with respect to a workplace located at 6330 Fountain Square Dr. Citrus Heights, CA 95621.
