29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof, Residence - On or about June 17, 2018, employees unloading new shingles on the roof of an existing one-story residence were exposed to fall hazards up to approximately 10 feet, 5 inches in that the workers were not fall protected. b) Roof, Garage - On or about June 17, 2018, employees removing shingles from the roof of an existing one-story garage/storage structure were exposed to fall hazards up to approximately 11 feet, 6 inches in that the workers were not fall protected.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite - On or about July 17, 2018, employees were not instructed in the hazards associated with their work duties including but not limited to fall protection requirements when working on residential roofs, thus exposing employees to fall hazards up to about 11 feet, 6 inches.
