29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Third Floor Balcony, Building C - On or about July 18, 2018, employees were permitted to walk/work at the edge of an open-sided balcony without being fall protected, thus exposing the employee to fall hazards of up to approximately 21 feet, 3.75 inches. b) Second Floor Balcony, Building B - On or about July 18, 2018, an employee was permitted to walk/work at the edge of an open-sided balcony without being fall protected, thus exposing the employee to fall hazards of up to approximately 10 feet, 7.9 inches. c) Third Floor Balcony, Building B - On or about July 18, 2018, an employee was permitted to work at the edge of an open-sided balcony without being fall protected, thus exposing the employee to fall hazards of up to approximately 21 feet, 3.75 inches.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5): a) Throughout Jobsite - On or about July 18, 2018 and times prior to, the employer did not ensure that all employees who operated powered industrial trucks (PITs) had successfully completed the training required by this paragraph (l) before allowing the employee(s) to operate a PIT, thus exposing employees to struck-by hazards.
