29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the hole. a) Floors 2 through 12 space adjacent the stairwell in the front portion of the building: On or about July 18, 2018, WDG Construction Group, Inc. failed to ensure that contractors on the site, were protected from fall hazards of between 10ft and 100ft while working around floor holes present on each level of the structure that were either not covered, not effectively covered or not protected by a guardrail system and employees working around the holes were not using a personal fall arrest system.
