29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a. Uni-Grip, Inc. - Upper Sandusky, Ohio: On or about July 16, 2018, the employer did not ensure that employees who setup the ESI catapuller machine (s/n 1522990) were trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that setup the equipment were not trained to control the machine's hazardous energy.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Uni-Grip, Inc. - Upper Sandusky, Ohio: On or about July 16, 2018, employees were exposed to pinch point and rotating part hazards in that the ESI catapuller machine (s/n 1522990) was not deenergized prior to servicing tasks such as, but not limited to, setting up the machine.  The employer did not ensure that energy control application steps were implemented to control the machine's electrical/mechanical energy source prior to setting up the machine; the employer did not:  - Ensure that equipment was turned off or shut down.  - Physically operate energy isolating devices to control energy sources and affix lockout device(s).  - Verify that isolation and de-energization of the machine or equipment had been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Uni-Grip, Inc. - Upper Sandusky, Ohio: On or about May 3, 2018, employees that operated the Engineering & Industrial Services Belt Puller machine (s/n 10598) were exposed to injuries from the ingoing nip-point created by the machine's rotating belts.  The employer did not ensure the machine was adequately guarded to protect employees from the rotating parts while feeding and/or adjusting the part during operation.     b. Uni-Grip, Inc. - Upper Sandusky, Ohio: On or about July 16, 2018, employees that operated the ESI Catapuller machine (model ASC Puller-Cutter; s/n 1522990) were exposed to injuries from the ingoing nip-point created by the machine's rotating belts.  The employer did not ensure the machine was adequately guarded to protect employees from the rotating parts while feeding and/or adjusting the part during operation.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a. Uni-Grip, Inc. - Upper Sandusky, Ohio: On or about July 16, 2018, the employer did not ensure that the shear point of the ESI catapuller machine (model ASC Puller-Cutter; s/n 1522990) was guarded.  The shear point, created as the cutting knife operated, exposed employees to amputation injury.      b. Uni-Grip, Inc. - Upper Sandusky, Ohio: On or about July 16, 2018, the employer did not ensure that Engineering & Industrial Services Belt Puller machine (s/n 10598) was guarded to protect employees from the pinch point created by the moving and rotating drill bit that applies a hole to part #168.    Uni-Grip, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1266775, citation number 1, item number 2 and was affirmed as a final order on February 21, 2018, with respect to a workplace located at 9823 State Hwy. 53 North, Upper Sandusky, Ohio 43351.
