29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)Dynasty Construction, LLC at 3640 Black Powder Road, Rapid City, SD 57703: On or before and at times prior to July 19, 2018 the exposing employer did not ensure employees that were working engaged in framing activities on a residential single-family home under construction, were protected from falls using a fall protection system. This condition exposed employees to a fall hazards between 10 and 19 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a)Dynasty Construction, LLC at 3640 Black Powder Road, Rapid City, SD 57703: On or about and at times prior to July 19, 2018, the exposing employer did not ensure that each employee engaged in framing operations on a single-family home under construction, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The condition exposed employees to the hazard of falling from an elevations of approximately 10 to 19 feet.
