The most recent occurrence of this was observed on July 24th, 2018 at a new residential home located at 20914 Ash St. Gretna, NE where the employer has a crew of 3 employees installing roof shingles exposed to fall hazards while they were working off the roof of the home without an adequate form of fall protection.
