1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) New residential construction located on Lot #8, Front gable roof: On or about 07/19/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that no protective eyewear was provided to employees installing Hardie-Plank siding products.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) New residential construction located on Lot #8, Front upper gable roof: On or about 07/19/2018, and at times prior, the employer exposed employees to fall hazards of 23-feet, 6-inches while working from a 8/12 pitched roof, in that a fall protection system was not provided to an employee engaged in siding installation on a steep roof. (b) New residential construction located on Lot #8, Front lower roof: On or about 07/19/2018, and at times prior, the employer exposed employees to fall hazards of 12-feet, 10-inches while working from a 6/12 pitched roof, in that a fall protection system was not provided to an employee engaged in siding installation on a steep roof.
