29 CFR 1910.23(b)(9): Ladders were not inspected before the initial use in each work shift, or more frequently as necessary, to identify any visible defects that could cause injury: Evanston Work Site - On or about July 21, 2018, the employer did not ensure that the 12 foot extension ladder used to climb a tree at the work site was inspected before initial use or more frequently as necessary to identify any visible defects that could cause injury. The ladder used at the work site by an employee had three bent rungs.
